Millennium Courtyard Order Form

Please fill in the order form bricks with the desired letters and emblem number(s). The engraving will be exactly as you specify. The emblems are shown actual size - some are horizontal, some vertical, some larger, and some smaller. The order form reserves space for the large vertical/horizontal emblems. The smaller emblems require half the space of the large emblems. Design your brick to fit the emblem.

If you desire a one or two line message, use the provided order forms.

The Brick engraver will center the message.

Order form for brick with ONE emblem, please fill in the boxes exactly as you want the engraving. Indicate, by number, which emblem you want. The last two letter boxes of line one are not available if you are using an emblem.
