Forms All Athletes Need
Prior to participation, each student athlete must have a valid Virginia High School League (VHSL) Pre-participation Physical Examination on file with the school.
An Emergency Care Card (ECC) must also be completed and provided to the coach. In the event any contact information changes, such as work phone number or insurance information, please complete a new ECC and return it to the school's athletics department.

You must file a completed Extracurricular Participation Policy form every season you plan to play sports.
An Assumption of Risk (i.e., the risk of the skin infections associated with participation in sports) form is required for participation in sports. It must be filled out by every student athlete (and parent or guardian) every season.

If your child participates in a sport sponsored by the Fairfax County Schools and you have no insurance coverage you will need to purchase one of the coverages explained on the flyer below. This is an EXCESS POLICY and will pay for only those covered charges in excess of expenses paid by any other insurance up to the maximums shown below. Even if you do have other insurance, this would help with co-pays and expenses not covered by your primary insurance.

Return to Play After Injury
If you want to compete after visiting the doctor following a sports-related injury, Fairfax County requires you to have the appropriate medical release forms signed by your doctor before you can return to competition.
FCPS and the Virginia High School League (VHSL) have developed several policies and guidelines designed to provide the safest environment for athletic participation. These include a return-to-play or "Care of the injured athlete while participating in FCPS Athletics" policy.
Asthma
Asthma is a condition that impacts the respiratory system of many high school students. When managed properly, asthma may have very little impact on the student's ability to participate in athletics. FCPS permission forms for athletes who use an inhaler must be provided to the school regarding the use of inhalers during school and athletic events. It is also important that athletes follow the dosage recommendations prescribed by the physician during athletic participation. Misuse of any prescription medication may cause disqualification from athletic participation. When prescribed, the inhaler MUST be present at all practices and contests.
Bee Stings
Bee sting allergies can pose a threat to athletes during practice and competition outdoors. If your child has been prescribed an Epi-Pen to prevent anaphylaxis, please complete the Epi-Pen Form and submit it with a spare Epi-Pen to the ATC at the school the first day of athletic participation.

Casts
At this time, VHSL rules limit participation with a cast to football and soccer players only, where the cast is below the elbow. If you feel it is appropriate for an athlete to continue participation in sports during the later stages of fracture healing, the appropriate cast release form must be completed by the physician and kept on record with the school's ATC.

Skin Lesions
Wrestlers should see the Athletic trainer if they have a skin lesion. The trainer may give the athlete a Wrestling Skin Lesion Clearance Form.

If you missed the presentation by the athletic trainers at the last Meet the Coach session, please watch the slide show.
If an athlete goes to a physician, the ATCs strongly suggest that he or she brings the Physician Response Form or a note from the physician back to the ATC to ensure appropriate treatment and rehabilitation of the injury.

Students Driving Other Students
Any student planning to drive, or be a passenger, to and from a sports event or practice needs to complete the Student Driving Activities (PDF) form and submit it to her or his coach. (Please notice this form refers to the FS-142 Field Trip Driver's License and Vehicle Insurance Information (PDF) form, which must also be completed and submitted.)
